Support Worker Job Description Purpose of the role: To enable people with Learning disabilities , mental health and associated needs to improve and maintain the quality of their life by: · Support in achieving daily living skills. · Identifying and meeting the needs of individuals. · Developing and maintaining relationships with service users, staff and the local community. · Positively supporting service user rights, choices, social inclusion and independence in keeping with Valuing People. Reports to: Business/Team Manager. Specific Responsibilities: · Working as part of a team in order to provide personal support for individuals with a learning disability within their own homes. · To provide opportunities for individuals to develop skills in a way that respects the person involved, promotes independence and allows individuals concerned to realise their own potential. Special Conditions: You may be required to: 1. Work shifts on a staffing roster including weekend, sleep-in and night duty as required. 2. Ensure staffing shortfalls are fulfilled on a flexible basis. 3. Cover special events such as service user holidays. 4. Attend staff meetings and review on a regular basis. Key Result Areas: 1. Assisting with providing a needs - led service. 2. Participating and assisting with defined support plans encouraging individuals to take an active role in all aspects of daily living. 3. Encouraging and supporting a lifestyle which facilitates the spiritual, physical, Psychological, cultural and social needs of the individual. 4. Assisting with the development implementation, review and evaluation of individuals programme plans within a multi-disciplinary framework. 5. Being responsible for and assisting service users with all aspects of household tasks as and when required. 6. Enabling individuals to develop and maintain relationships within their home and the local community. 7. Reporting any service needs of whatever nature to your line manager. 8. Ensuring that all relevant documentation is completed and accurate. 9. Providing contingency cover at other establishments, if required. 10.
