Role Profile Temporary until 31st March 2018 ROLE DEFINITION Anam Cara is a Dementia Respite Unit offering flexible residential breaks for up to 14 adults with a diagnosis/probable diagnosis of Dementia Care Practitioner duties will include personal care, assessment, care planning and review, activity co-ordination for Anam cara guests and training of care assistants.  To work as part of a team of staff to ensure guest's and carer's needs are met. To promote person centred care plans and liaise with all other multi-disciplinary colleagues KEY TASKS AND RESPONSIBILITIES Corporate Responsibilities * To undertake individual role in line with the Council’s Strategic Priorities and in compliance with the Council’s corporate policies and procedures. * To undertake any other reasonably required duties as instructed by Management or someone acting on their behalf, in addition to the role specific tasks & responsibilities detailed below. Role Specific Tasks & Responsibilities * To work as part of a team to ensure service user needs are met through Person Centred Care Planning * Requirement is to work 12 hour shifts, 15 shifts over a 5 week period to include day shift and night shift * To use the keyworker system in the admission of guests and support of carers * To carry out continuous assessment care planning and review of activities and daily living tasks * To meet and maintain your registration requirements of the SSSC * To work in compliance with National Care Standards * To comply with NAHSCP policies and procedures * To undertake appropriate training and train care assistants in best practice    * To undertake any other duties as directed by the Manager  ESSENTIAL / DESIRABLE CRITERIA     Essential Desirable Evidence Education and Qualifications * SVQ 3 in Social Care or equivalent health qualification * SSSC Registration process or health equivalent   * Evidence of self development * Dementia Specific Training or Qualification     * Application Form * Interview Experience * Previous experience of working with people with Dementia * Assessment Care Planning Review experience      * Inter-agency working * Person Centred approach to care * Working with Carers * Administration of medication     * Application Form * References * Interview Specialist Knowledge * Knowledge of Dementia and associated health needs * Knowledge and understanding of National Care Standards    * Knowledge of Best Practice for Dementia Care * Promoting Excellence skilled level Dementia Practise * Application Form * Interview * Pre/Post-Interview Check (if appropriate) Skills and Abilities * Ability to work autonomously and as part of a team * Excellent communication skills * IT skills  * Leadership skills * Report writing skills   * Problem solving skills    * Application Form * References * Interview Other * Must be articulate and of smart appearance * Flexible and responsive to guest and carer's needs  * Must be able to work rotational 12 hour shifts including weekends * Must be fit enough for CALMs and M&H training   * Person centred approach to delivery of care     * Application Form * Interview * Pre/Post-Interview Check (if appropriate) Staff Values & Behaviours North Ayrshire Council uses behavioural based approach throughout the recruitment process.
