Role Profile Temporary until 31st March 2018 ROLE DEFINITION Anam Cara is a Dementia Respite Unit offering flexible residential breaks for up to 14 adults with a diagnosis/probable diagnosis of Dementia Care Practitioner duties will include personal care, assessment, care planning and review, activity co-ordination for Anam cara guests and training of care assistants. To work as part of a team of staff to ensure guest's and carer's needs are met. To promote person centred care plans and liaise with all other multi-disciplinary colleagues KEY TASKS AND RESPONSIBILITIES Corporate Responsibilities * To undertake individual role in line with the Council’s Strategic Priorities and in compliance with the Council’s corporate policies and procedures. * To undertake any other reasonably required duties as instructed by Management or someone acting on their behalf, in addition to the role specific tasks & responsibilities detailed below.
