An exciting opportunity has arisen within the congenital heart disease transition team at the Royal Brompton &Harefield NHS Foundation Trust. We are seeking a highly motivated and dynamic nurse to join our specialist nursing team as a Band 7. You will work within the multidisciplinary team and support young people between the ages of 12 and 25 years with congenital heart disease through the transition process as they prepare to transfer to the ACHD unit and to support them once they have successfully transferred. The congenital heart disease transition service works closely with the paediatric cardiac nurse specialist team and also the adult congenital heart disease CNS team and offers a bridge between the two services. You will assist with the co-ordination of medical care for this patient group as well as running the nurse-led congenital heart disease transition clinics and offering advice and support via telephone, email, and during hospital admissions. At the Royal Brompton &Harefield NHS Foundation Trust we are committed to your personal development and this will be facilitated by our Senior Nurse in ACHD, Practice Educators, CNS colleagues, and our Medical Team. Access to academic courses and conferences is encouraged and financial support and study leave can be applied for through the learning and development team. We also support nurses to participate in service development, research and audit. As a member of the nurse specialist team you will also be invited to join the trust’s Advanced Nursing Team and gain further support and knowledge from other members of this team.
